Criteria


Seton Medical Center and Seton Coastside are pleased to announce the 2018 Community Grants Program.  Specific information is outlined below for non-profit organizations who serve Seton and Seton Coastside’s service area.

  • Funding will address one or more prioritized significant needs in Seton’s and Seton Coastside’s service area (Daly City, South San Francisco, Pacifica, San Bruno, Half Moon Bay, Brisbane, Moss Beach, El Granada, Montara and San Francisco within zip codes 94112, 94132, 94134)
  • People to be served include identified vulnerable or underserved populations, to help address health disparities identified in the Community Health Needs Assessment (CHNA) and Implementation Strategy report and are one of Seton Medical Center’s top five priorities:
    1. Obesity and Diabetes (including fitness and nutrition)
    2. Access and Delivery
    3. Behavioral health
    4. Heart Disease and Stroke
    5. Cancer

The 2018-2019 grant application period is open from January 8, 2018 to midnight on February 28, 2018. Grants will be announced on April 1, 2018. 

Eligibility and Parameters

  • Applicant must be a valid 501(c)(3) non-profit organization
  • Grant awards are from $1,000 to $ 25,000. 
  • Implementation project performance period is due July 1, 2018.
  • Final performance evaluation is due December 31, 2018.
  • Funding must address the need and impact of the on the community or patient population served in Seton Medical Center’s and Seton Coastside’s service area.
  • Grant funding may be part of an existing project. 

Applying for a Grant

Please submit no longer than a two page request with the following information: 

  • Brief Background on the Non-profit organization
  • Need for Funding and Impact of your work
  • Overview of project budget
  • Brief biographies on staff involved with project
  • Evaluation tools demonstrating success
  • Sustainability of project post-funding
